POSITION: Advanced Practice Nurse in Primary Care and/or Family Medicine
LOCATION: Mobile Medical Clinic in Hartford CT.
ASSIGNMENT: Full Time, 35 hours per week with Salary and benefits, Monday-Thursday (Friday availability may be required), as needed, as an independent contractor providing adult primary care
History and Background
Since 2006, the mobile clinic has been providing high-quality, free primary care to uninsured adults in Greater Hartford. Many now consider our big white van as their only medical home. Patients are seen in a safe, non-threatening environment at four church sites in Hartford and East Hartford. Specific van locations and hours are generally:
* Monday, 9am-5pm, East Hartford
* Tuesday, 1-6pm, Farmington Avenue, Hartford
* Wednesday, 10am-2pm, Farmington Avenue, Hartford (as of March 2019: 9am-1pm, Capitol Avenue, Hartford)
* Thursday, 12-5pm, Campfield Avenue, Hartford
By year-end 2018, the clinic provided 51,551 free patient visits (5,489 visits in December alone). Our patients have the common chronic diseases like diabetes, hypertension, hyperlipidemia, and asthma. They lack a primary care provider because they are unable to qualify for state or federal assistance and are unable to afford the cost of health insurance with its premiums, deductibles, and co-pays.
Our patients tend to postpone treatment for routine health care and in the past would have resorted to using emergency rooms for routine care. Many have had intermittent or inconsistent treatment, which can lead to long-term complications.
Our mission is to provide high-quality and consistent primary care. We try to impress upon our patients the need for routine follow-up and are working on projects to achieve that goal.
Provide direct patient care in our mobile van or office with a typical patient load of 7-12 patients per afternoon.
Discuss lab/x-ray/ultrasound results and plan of action for follow-up care.
Reconcile medications and remind patients to refill their meds in a timely manner.
Discuss with patients any impediments that may be keeping them from getting labs, x-rays
or meds to improve their health.
Develop a treatment plan and work with our volunteer medical professionals as needed.
